TO OBTAIN A COPY OF YOUR SACRAMENTAL RECORDS FROM ST. JUDE

To obtain a copy of your birth certificate or confirmation verification for Sacramental purposes (ie: First Communion, Confirmation or Weddings),

please send an email to Avril Allori.

In your request please include the following:

• Your full name at birth and date of birth
• the date of Sacrament, if known; a best guess of year or year range is very helpful
• name and address where you would like the baptismal record sent
